Job Description
The Ambulatory Care
Coordinator, a member of the care management team, works with outpatient
members, providers and caregivers deliver supportive services, under the
direction of the Ambulatory Care Management (ACM) team to increase efficient
utilization of services. Research and provide healthcare resources and on
demand information to patients and staff under direction of the ACM team. Services
are performed face to face or telephonically from the Care Management
Department. Serves as a liaison between providers, patients and care management
teams. All interactions are documented and sent to appropriate individuals.
Provides reports as requested.
Qualifications
High School Diploma or
equivalent. AA or Bachelor’s Degree in healthcare related field or current
equivalent related work experience preferred. Two years office or
administrative experience required. Basic knowledge of Medical Terminology
preferred. Working knowledge of guidelines and regulations relevant to case
management and utilization management, including Medicare, DMHC, NCQA, and
Milliman Care Guidelines.
