Job Description

The QI Transformation Facilitator job family is comprised of differing types and levels of interactions with internal and external clients to provide them with appropriate and timely service and interventions. A QI Transformation Facilitator is a trained individual who provides high quality and cost effective initiatives to the site management team and supports the site operations team in the implementation and sustainment of the Practice Improvement Model. 



QI Transformation Facilitator I - High School Diploma, GED or Proficiency Exam. One year of experience in a medical, managed care of health care setting. QI Transformation Facilitator II - Associate Degree. Bachelor’s Degree is preferred. Three years of experience in a medical, managed care or health care setting. Must be proficiently skilled in the application and use of Windows, Word, Excel, Outlook and the Internet.  Ability to develop and maintain a key-contact database with mail merge capabilities or utilize other computer resources to assist with monthly reports and other correspondence.  Excellent written, verbal and interpersonal communication skills.  Basic knowledge of Medical Terminology and working knowledge of ICD/CPT codes. Ability to maintain composure when confronted with fast-paced, stressful situations; to multi-task in an efficient, thorough, and prioritized manner. Lean Management or CPHQ preferred.
