Job Description

The QI Transformation Facilitator II is responsible for working with patients/members, internal and external clients to provide them with appropriate and timely service in order to improve overall patient care, achieving higher compliance and performance level. Assists with the clinic management in sustaining the new care delivery model by using the knowledge, skills and tools of team-based care to assist practices to install, apply, and improve this new delivery mode and is responsible for continuous rounding at assigned sites to monitor quality performance improvements demonstrated in the quality measures such as CMS 5-Star, P4P, QPP, PT. Experience, etc.



Associate Degree. Bachelor’s Degree is preferred. Three years of experience in a medical, managed care or health care setting. Must be proficiently skilled in the application and use of Windows, Word, Excel, Outlook and the Internet.  Ability to develop and maintain a key-contact database with mail merge capabilities or utilize other computer resources to assist with monthly reports and other correspondence.  Excellent written, verbal and interpersonal communication skills.  Basic knowledge of Medical Terminology and working knowledge of ICD/CPT codes. Ability to maintain composure when confronted with fast-paced, stressful situations; to multi-task in an efficient, thorough, and prioritized manner. Lean Management or CPHQ preferred.
