Job Description


The Pharmacy Technician will, under the supervision of an Advanced Practice Pharmacist (APP), utilize specialized pharmacy knowledge and expertise to provide integrated support to EPIC medical group clinicians and patients to ensure efficient, timely, and effective medication reconciliation for our members, especially during the immediate post-hospital discharge period. Working under the guidance of the APP and in coordination with the Transition of Care (TOC)/Post Discharge LVN this position will primarily be responsible for the screening, the triage, and the pharmacy administrative support of all patients for medication reconciliation in the immediate post-discharge period. The Pharmacy Technician will also coordinate Tele-Health consults as needed.




High School Diploma or equivalent, college courses desirable. Complete higher education coursework such as an Associate’s Degree in Pharmacy technology, completing a course accredited by the American Society of Health-System Pharmacists, or earning a degree from any pharmacy school accredited by the American Council on Pharmaceutical Education is preferred. One year pharmacy technician experience required. Two years pharmacy technician experience preferred. PTCB certification required or can submit proof (form DD214) that training was completed while in the federal armed services.
