Job Description

The Care Navigator (CN) will increase member’s access to high quality cancer care. The CN will function as a member of the interdisciplinary team and serve as an advocate and educator for cancer patients. The CN enhances members’ ability to manage their oncology disease process, by conducting care management activities; including assessments, planning and implementing interventions, and communicating actionable member data to the health provider. The employee will be working telephonically and in the field, attending doctor’s visits with the member. The Care Navigator will conduct home visits, assist the members with finding community resources, and help them navigate the complex healthcare system.


Bachelor’s Degree in Health Care, Health Administration, Health Education, or similar field required. Three years in the medical field with a Bachelor’s Degree; OR one year in the medical field with a Master’s Degree in a Health Care, Health Administration, Health Education, or similar field. Previous health care experience in customer service or medical management support. Experience with motivational interviewing preferred. Working knowledge of guidelines and regulations relevant to case management and utilization management, including CMS, DMHC, NCQA, and Health Care Guidelines. Strong skills to comprehend and identify member needs and barriers. Excellent written and verbal communication skills. Ability to communicate effectively.
