Job Description

The Ambulatory Care Social Worker I is a member of the care management team, under the auspices of the Director of Ambulatory Care Management and Ambulatory Care Supervisor.  The Social Worker I provides managerial direction for Community Health Workers and serves as the resident expert with all behavioral health matters for the Medical Management Department.   Provides assistance with outpatient members, providers and caregivers to facilitate intensive, comprehensive care management and increase efficient utilization of support and community services. The Ambulatory Care Social Worker I will conduct escalated service coordination on social and social determinant complex cases and monitor through resolution, according to program guidelines. The Ambulatory Care Social Worker I is responsible for assessment and intervention to meet the social determinant needs of Care Management patients and their families/caregivers in the outpatient setting. This position includes advocacy, research, education, and any other assigned responsibilities relating to Care Management social work.  The position requires a significant degree of independence and the ability to be the subject matter expert for the department and make professional decisions and communicate to the provider, nurses and other members to the care team.   Services are performed face to face or telephonically from the Care Management Department. The Ambulatory Care Social Worker I serves as a liaison between providers, patients, community and public resources and care management teams. All interactions are documented and sent to appropriate individuals. Provides reports as requested.



Graduation from an accredited college with a bachelor in social work or related field. Master’s degree in social work or related field preferred. One year of experience working in healthcare, or in providing social services. Two or more years of experience in an IPA/Medical Group or health plan in a managed care environment. Excellent customer service skills. Demonstrated ability to utilize/apply the general and specialized principles, practices, techniques and methods of case management. Bilingual skills desirable. Understand confidentiality and the legal and ethical issues pertaining to case management. Knowledge of available community resources. Proficient computer skills and using Microsoft Office applications. Strong written and verbal communication skills.
